Support to reduce re-offending

SWIM works directly with ex-offenders from ACH communities and other people at risk of entering the Criminal Justices System and our referrals come from probation service providers.

PACT’s focus on the generational differences between older black men and their children and grandchildren’s generation, helps to provide perspective and acceptance. This equips participants to revisit and rebuild relationships positively. The goal is to empower people to become strong role models in family and social circles.

Social inclusion & employability, Drug and alcohol support plus mental health outreach

ACH communities

SWIM provides psychosocial interventions to people from ACH communities with diagnosed and undiagnosed mental health challenges. We also engage with hundreds more people through our outreach work as we raise awareness of health issues and improve access to primary care services for the communities we represent. Our referrals and funding come from clinical commissioning groups, GP federations and local councils. We help build capacity and skills to deliver health and well-being services within other like-minded third-sector community organisations.

Peter was responsible for implementing the update EMIS and SystmOne clinical system to accept 111 direct appointment booking during surgery hours from 111 providers. Peter displayed numerous strengths including excellent communication and engagement skills with a range of stakeholders including Local Medical Committees, clinical leads, practice managers and senior management teams. His core skill is his work ethics and ability to overcome challenges and obstacles.
